Reports & Documentation

Practical report templates for citations, arrests, pursuits, use of force, collisions, and complaints, and when a report is required.

Reports are how NHP keeps a record of what happened. File them through the server's actual reporting method (the NSRP Discord unless command has set another system).

When a report is required

·Every traffic collision NHP works.
·Every arrest.
·Every pursuit, including terminations.
·Every use of force, no matter how minor.
·Every collision involving a trooper or a patrol vehicle.
·Any incident involving a complaint or a policy violation.

Reports are filed before the shift ends.

Traffic citation and warning template

Citation / warning report fields

Date and time	When the stop happened
Location	Road and nearest landmark or junction
Trooper unit	Your unit number
Vehicle and plate	Description and plate status
Driver	In-game name of the driver
Violation	The observed violation
Outcome	Warning or citation
Notes	Driver behavior, any refusal, anything unusual

Arrest report template

Location	Where the arrest took place
Trooper unit	Your unit number
Suspect	In-game name
Charge	The lawful charge stated at arrest
Reason	The roleplay facts that support the charge
Force used	None, or the specific force and why
Transport	To booking, time, any second unit
Outcome	Booked and the final result

Pursuit report template

Date and time	When the pursuit started
Initiating unit	Your unit number
Reason	Why the pursuit was justified
Suspect vehicle	Description and plate if known
Path	Direction and major roads or junctions
Tactics used	Spikes, roadblock, PIT (with supervisor approval)
Supervisor	Who oversaw the pursuit
Outcome	Apprehended, escaped, or terminated
Damage or injuries	Any collisions, injuries, or civilian impact

Force used	Taser, firearm, physical, and the level
Proportionality	Why this force matched the threat

Location	Where on the road
Trooper unit	Your unit number
On duty	Whether you were on a call or routine patrol
Vehicles involved	Your patrol car and the other vehicle(s)
Injuries	Any injuries to you or others
Cause	What led to the collision
Outcome	Scene handling, report, any discipline referral

A collision involving a trooper is always reported to a supervisor, even a minor one.

Complainant	The player or trooper reporting
Summary	What happened, in facts only
Policy reference	Which policy or rule is in question
Witnesses	Other units or players who saw it
Action requested	Supervisor review, mediation, or discipline referral

Use only fields relevant to the roleplay. File through the NSRP Discord or the method command has set.
